Sending Applications to the Ontario Ventilator Equipment Pool During the Covid-19 Pandemic

As per the memorandum dated March 24, 2020 from the Ontario Ministry of Health:

The Application for Funding Ventilator Equipment and Supplies  along with the Addendum for Ventilator Equipment and Supplies Application Form, are to be sent directly to the Ontario Ventilator Equipment Pool (VEP), located at Kingston Health Sciences Centre.

Do not send the Application and the Addendum to the Assistive Devices Program (ADP).
Applications sent to ADP will not be processed.

The Application and the Addendum must be sent by email to: VEPApplication@kingstonhsc.ca.

Upon receipt of the documentation, the VEP will notify the sender that they have received the documents.

As a reminder:

• the VEP will not process any Application that is incomplete or does not include the Addendum; and
• the Addendum should always include the name and contact information of the health care professional the VEP can contact for additional information or clarification.
